CHRONIC DISEASE MANAGEMENT

MEDICARE REBATES are available for people with Chronic Medical Conditions

Chronic Medical Condition (definition as defined by Medicare)

A chronic medical condition is one that has been (or is likely to be) present for six months or longer.  It includes, but is not limited to conditions such as asthma, cancer, heart disease, diabetes, arthritis and stroke.  Your doctor will determine whether a care plan is appropriate for your condition.

GP Management Plan (GPMP)

This is a plan that you and your G.P. organise to address all your health needs over the next 12 months.  Therefore it includes planning for bloods, procedures, referrals and radiology that you may need.

Team Care Arrangement (TCA)

Additionally, people with complex care needs may be eligible for a Team Care Arrangement.  In light of you being eligible for a TCA you will receive subsidised Allied Health services.

To be eligible for a TCA your doctor MUST refer you to at least 2 other health care providers. Allied Health providers and specialist qualify as other health care providers. What’s more services by these providers must relate to your chronic condition.

Booking Appointments to arrange a GPMP or TCA

GP Management plans and Team Care Arrangements take some time to organise.

When booking your GPMP or TCA appointment it important to inform reception that an extended appointment is required.  This is to ensure the doctor is able to allocate the appropriate time to complete a GPMP or TCA & also to ensure efficient running of the practice is continued. The doctor may ask you to return at another time if the appropriate appointment time is not scheduled. 

In general, Chronic Disease Appointments are bulk billed.
