The surgery does not used patient’s health information for any other purposes without their consent.

There are some necessary purposes for collection of information beyond providing health care. Such circumstances are professional accreditation, quality assessments, clinical auditing, billing and so forth.

All patient information is in a form that is accurate, complete and up to date. Information held by the practice is relevant to providing health care.

After your last attendance the practice is required by law to keep your patient records for a total of seven (7) years. It is also prudent having regard to administrative requirements.


Your medical record is a confidential document. It is the policy of this practice to secure personal health information. In addition health information is only available to authorised members of staff.

Access and correction

Patients may request access to their personal health information held by this practice.

As a matter of fact patients have the opportunity to amend any incorrect personal information held.

There are some circumstances in which patient access is restricted. However, in these cases reasons for denying access will be explained.

Additionally a charge may be payable where the practice incurs costs in providing access

These are the numbers, letters or symbols to identify patients with or without the use of 2 names. (eg Medicare numbers). As a result the practice will limit the use of identifiers assigned to patients. But, Hobsons Gate Medical Centre will fulfil their obligations to Commonwealth Government agencies.


A patient has a right to be anonymous, provided this is lawful and practicable. In the medical context this is not likely to be practicable or possible. Medicare and insurance rebate payments need known patients. Also it could be dangerous to the patient’s health.
